Registration Forms
Registration Forms may be downloaded from
this website. There are two versions of the registration form.
If you are a participant, whether you are a youth or an adult,
please use the CREDO PARTICIPANT Liability Release Form. If you wish to
serve as a chaperone, please use the CREDO CHAPERONE Liability Release Form. All information must
be completed, including Medical Information, and if under 18,
signatures of both a parent and a chaperone. The cost of the
three day weekend is $35.00 per person for both participants
and chaperones. Registrations received by September 5th will
receive a FREE t-shirt!
Chaperone Requirements
Review the CREDO Chaperone Liability Release Form in its entirety. Please
note that a chaperone is limited to seven (7) youths, and must
have a pastor or appropriate parish representative sign to verify
the chaperone has gone through the child protection training
in accordance with the Archdiocese of Cincinnati policy. Chaperones
are a very important part of making the event a safe and spiritually
enriching experience. We take the role and responsibility of
the Chaperone seriously, and we will try to accommodate any
special concerns you may have with all our attendees. Please
contact us by email, phone, or at the event. We thank you for
your commitment to sharing your faith with our youth. Chaperones
may also optionally serve as small
group leaders for our two Saturday breakout sessions (see
below).
Small Group Leaders
Small Group Leaders are also a very important
part of making the weekend reflective, spiritual, and faith

Food and Refreshments
JTM Food Group has generously supported the
Youth Retreat for many years. There will be light snacks on
Friday evening; a full lunch (Arby's, etc) and pizza for dinner on Saturday; plus
snacks on Sunday morning. Beverages are provided.
Safety and Security
CREDO hosts have the greatest
interest in the safety and security of all attendees. There
will be security on premises at all times, with assistance from
the Hamilton County Sheriffs' Department. No one will be permitted
into the program without appropriate identification and chaperones.
Parking will be restricted to participants only, with pick up
and drop off areas designated. Please cooperate with the Safety
and Security Team.
